What is a Shared Living Provider?

A Shared Living Provider (SLP), once known as an Extended Family Home (EFH), is a program wherein individuals with developmental disabilities live in a private home with a person, couple, or family. A Shared Living Provider has chosen to share life with individuals by providing; a healthy home, positive support system, community inclusion, transportation, and also helps oversee medical appointments. It is a team-based program, giving individuals with developmental disabilities, options to be embraced in family-based homes, while still receiving needed services for them to succeed in life. 

 

Anew Shared Living Providers strives to foster a support system that encourages individuals to meet personal goals, and guidance for SLP's to assist in their success!
